Can my mate die/be permanently stuck somewhere?

No, your mate cannot permanently die. Just like your main wolf, if your mate is killed, he/she should respawn.
If your mate is stuck somewhere (like an 'invisible wall'), first try howling. If it isn't responding to you, just save and end the game, then reopen that saved game. The issue should be fixed.
