Can I send my COPPA form through e-mail?

Due to federal regulations, we are required to have parent/guardian permission for any WolfQuest member in the United States who is younger than 13 years old. Unfortunately, we are not able to accept COPPA permission forms through e-mail because we are required to have the actual guardian signature. If you live in the United States, you can print out the form, have your parent/guardian sign it, and either fax it to the number listed on your form or mail it to the address listed below.

If you do not have access to a printer or no longer have the COPPA form, please clearly write down on a piece of paper the following and mail it to the address listed above:
  • your username
  • your e-mail address
  • your parent/guardian's printed name
  • your parent/guardian’s signature
  • date of signature
  • the statement in quotes below
“I have reviewed the information provided by my child and hereby grant permission to "wolfquest.org" to store this information. I understand that I may request for this information to be removed from "wolfquest.org" at any time. I have read and understand the notice about online chat and safety.”
